Completion of this survey is required as part of the formal training outcomes assessment reviewed by the program’s accreditation body, the Commission on Dental Accreditation (CODA).

The purpose of the survey is to help the Dental Laboratory Basic Program faculty evaluate the program’s success in preparing graduates to function as competent dental laboratory technicians using a formal and ongoing planning and assessment process. Compiled data from all returned surveys will be used to evaluate program quality; data from individual surveys will be held in strict confidence.
