The Minnesota Department of Health invites you to register your upcoming diabetes prevention program offerings. From this registration, MDH will compile a list of programs and current class offerings across Minnesota and in neighboring states that will be available online at www.icanpreventdiabetes.org.
This information will also help MDH discover which parts of Minnesota have enough DPP classes and those areas that might need more offerings. It is our goal that all Minnesotans have convenient access to this program.
Participation in this survey is voluntary and will not affect your relationship with the Minnesota Department of Health. If possible, please have one person from an organization complete the survey.
DPP registration should take about 5 to 10 minutes, with an additional 2 to 3 minutes per class registration.
