Walkable Community Workshop Application
 
MDH will offer a limited number of Walkable Community Workshops in Minnesota in fall 2018. Communities that participate in Walkable Community Workshops will create a draft action plan, which is expected to be completed and implemented.
 
We look forward to hearing from you. Applications are due by 5 p.m. on Wednesday, June 6, 2018.
 
By submitting this form you are expressing interest that as a host you will help coordinate with the Minnesota Department of Health and assist in locating a suitable facility for the workshop; provide lunches or snacks and coordinate implementation of action plan. Hosts are responsible for identifying and inviting the right stakeholders to the workshop and will act as a follow-up contact.
 
Please provide contact information for your applicant/contact person and your co-sponsor(s).* One sponsor must represent public health and the second sponsor must be a transportation decision maker (public works, planning, engineer, etc.).
*If the applicant represents either public health or transportation, only one co-sponsor is needed.

 
NOTE: Completion of this form does not guarantee that your community will be selected to host a workshop. MDH and partners reserve the right to select communities based on identified needs, engagement and geographic distribution.
