Building Action Collaboratives for Health Improvement: A focused discussion of community needs

 
Please join us for a regional convening of Community Health Needs Assessment/Community Health Assessment (CHNA/CHA) practitioners.

Participants will:

-Build upon and strengthen local and regional collaborative relationships
-Share resources and current/best practices in completing Community Health Needs Assessments (CHA/CHNA)
-Discuss existing barriers and clarify technical assistance needs
-Identify opportunities for action-oriented collaboration to improve efficiency and effectiveness of local assessment and planning efforts. 

Desired participants: those directly involved in conducting the CHA/CHNA assessment, planning and implementation.

Dates and locations:

Metro - May 20th from 12 - 4 PM: MN Counties Intergovernmental Trust (MCIT), 100 Empire Drive, St. Paul, MN 55103

St. Cloud: May 24th from 12- 4 PM: 
Kelly Inn St Cloud, 100 4th Avenue S, Saint Cloud, Minnesota, 56301

Mankato: June 1st from 9 AM - 1 PM: 
Country Inn and Suites – Hotel & Conference Center, 1900 Premier Drive, Mankato, MN 56001
 

This convening is intended to build connections and partnerships at local and regional levels. Please attend the meeting closest to your location so that you can connect with others in your area.

Collaborators with state-wide focus, such as health plans and some health systems, are invited to participate.

All participating organizations are encouraged to limit your participation to 1 or 2 representatives in order to promote balanced conversation at the event.

A boxed lunch and a light breakfast or afternoon snack will be served at each event.
